History Taking: Establishing a Database

A thorugh assessment of an individual requires a detailed history taking. The precise, depth and extent of a nursing history varies with its intended purpose. Nurses use their judgment in deciding what data are necessary and relevant for the identification of problems. It provides the nurse with the information for identifying problems of psychosocial in nature of an individual.Further the history taking also provides information determining anticipatory guidance that an Individual may need. For the ill individual, it provides with the base line data for nursing diagnosis and nursing interventions.Assessing childs health begins with an interview with the parent. If the child is an adolescent or can participate in the interview actively both the parents and child may be interviewed.

Conducting an Interview

It is important as a matter of courtesy for a nurse to introduce herself and give a short explanation about the interview. You may refer sub-section of this block. After collecting the identification data, the reason for bringing the individual to the health care agency should be asked to the parents. Initially the client/parents may not come out with their real concern or fears. It is Important here to gain their trust. Once the client narrates the chief concern/problem ask him/her to describe at least six aspects of the problem (i) duration (ii) intensity,(iii) frequency, (iv) description of associated symptoms, (v) action taken and (vi) outcome. It’s important to know what the client/parents have done for their illness/problem. It’s important to ask ‘Is there anything else which worries you.

Before assessing growth and development, it is important to collect information about the family to know the environment in which the individual lives. It is important to assess the socio economic status and means of family support.A history of the pregnancy of the mother of the child being assessed is important. Assessment of the child at birth is important. Nurse must ask about any history of past illnesses. The individual’s present skills, nutrition, sleep patterns and interactions with this family can all be elicited by asking the parent to describe a typical day.Some diseases are inherited, it is important to know about family history of heart disease,diabetes mellitus etc. .

Communication and the Health Interview

Communication includes all those behaviours by which one person, consciously or unconsciously sends messages to one another which affect their behaviours.Communication is the most important skill used in assessment of an individual. It is the most important feature in forming trusting relationship.Communication may be verbal or nonverbal. Communication must be appropriate to the situation, properly timed and clearly delivered. It. must increase the understanding between themnurse and the client.

Interviewing is a specific goal directed communication. It requires time and patience to develop interviewing skills. Assuming privacy and informing the client’s about the confidential limits is important. Nurses must encourage the client to talk. She should be an active listener.
